THE SUMMARY:
That law requires doctors and other healthcare providers (including fire districts) to get written authorization from each patient before they can share most health information about him or her with a “third party” — and that includes most caregivers, even those who are close relatives. The form below will allow the release of records of patients for which Little Rock-Fox FPD has provided care.
